Verification of Incident Classification
The Verification of Incident Classification screen, which is part of the final section of the incident report, allows users to verify the incident categories and investigation determination that were documented in the first section of the incident report. If the incident categories and/or investigation determination have changed since submission of the first section, the user may make necessary modifications to the incident using this screen.
Criteria for Reporting an Incident to Adult Protective Services (APS): Incidents must be reported to APS when:
- The individual is 18-59 years old (based on the date of birth field), and
- The incident primary category is abuse, neglect or exploitation.
When both of these conditions are met, the user should report the incident to APS, and select 'Yes' to 'Was this incident referred to Adult Protective Services (18-59 years of age)?'.
Criteria for Reporting an Incident to Older Adult Protective Services (OAPS): Incidents must be reported to OAPS when:
The individual is over 60 years old (based on the date of birth field), and
- The incident primary category is abuse, neglect or exploitation.
When both of these conditions are met, the user should report the incident to OAPS, and select 'Yes' to 'Was this incident referred to Older Adult Protective Services (60+ years of age)?'.
Note:
Certified Investigators are not used by OLTL; providers conduct internal investigations to obtain information for the final section of the incident report.
Although this screen pre-populates with answers from the first section's Incident Classification screen, any changes that are made to this screen will update the overall incident, but will not change the responses that have been saved in the incident report first section.

Reason for Reclassification (if applicable) | If the primary or secondary category of the incident was changed, enter the reason for the reclassification. |
