Select the date the investigation ended. |
Investigation Type |
Select from the drop-down whether the investigation was an on-site visit, telephonic, file review, or other investigation. |
Description of Incident |
Enter a description of the incident. |
Investigation Action Taken |
Enter a description of the investigation action taken. |
Did any of the following outcomes occur as a result of the incident? |
Select the outcomes that occurred as a result of the incident. |
If other, please describe |
If 'other' was selected as an outcome of the incident, provide an explanation. |
