Investigation Information

The Investigation Information screen is used to record incident information collected as part of the investigation.

 

Field Name

Field Description

Investigation Start Date

Select the date the investigation started.

Investigation End Date

Select the date the investigation ended.

Investigation Type

Select from the drop-down whether the investigation was an on-site visit, telephonic, file review, or other investigation.

Description of Incident

Enter a description of the incident.

Investigation Action Taken

Enter a description of the investigation action taken.

Did any of the following outcomes occur as a result of the incident?

Select the outcomes that occurred as a result of the incident.

If other, please describe

If 'other' was selected as an outcome of the incident, provide an explanation.
