Investigation Action Taken

This is a configurable screen that allows users to record actions which were taken as part of an investigation.

Note: Mandatory fields are marked with an asterisk. You must complete all the mandatory fields on a screen. In addition, some fields become mandatory as a result of your answer to previous fields.

Field Name	Field Description
Were referrals made to other agencies?	Select Yes or No to indicate whether a referral was made to another agency.
If so, what agency were they referred to	Check the box to indicate which agency received the referral, if any.
Was the participant notified within 24 hours that a critical incident report has been filed?	Select whether the participant was notified within 24 hours that a critical incident report has been filed.
Was participant notified of the resolution and measures implemented to prevent recurrence?	Select Yes or No to indicate whether the participant was notified of the resolution.
If so, who notified the individual (name, title and agency)	If the participant was notified, enter a name, title and agency of the individual who notified them.
