Initiator Information

The Initiator Information screen displays read-only information about the user that filed the incident. All information is stored as of the time when the incident was created and is not editable by the end user.

Field Name	Field Description
Organization Type	This read-only field displays the type of organization that the initiator is associated with.
Organization	This read-only field displays the name of the organization that the initiator is associated with.
MPI (if applicable)	This read-only field displays the provider's Master Provider Index (MPI) number.
Initiator Name	This read-only field displays the name (last name, first name) of the user who created the incident report.
Phone Number	This read-only field displays the phone number of the user who created the incident report.
Email	This read-only field displays the email address of the user who created the incident report.
