Initial Action Taken

This is a configurable screen that allows users to record information about the actions taken in response to the incident.

Note: Mandatory fields are marked with an asterisk. You must complete all the mandatory fields on a screen. In addition, some fields become mandatory as a result of your answer to previous fields.

Field Name	Field Description
Type of Contact	Select the type of contact from the drop-down list, such as onsite, telephonic, etc.
Please describe the initial action taken	Enter information about the actions taken as a result of the incident.
Was the participant/victim separated from the target?	Select whether the participant/victim was separated from any target involved in the incident.
If no, please explain	If the individual/victim was not separated from a target involved in the incident, provide the reason why.
Were supports offered to the participant/victim?	Select whether the participant/victim was offered supports in response to the incident.
If no, please explain	If the individual/victim was not offered supports in response to the incident, provide the reason.
Medical Attention Given	Select the medical attention given (Assessment of Injury, CPR Administered, Emergency Room, First Aid, Hospital, Primary Care Practitioner, Urgent Care Center, Other, Not Applicable).
If other, please describe	If ‘Other’ was selected as the medical attention given, describe what medical attention took place in response to the incident.
Was a call made to 911?	Select whether a call was made to 911 in response to the incident.
