Testimonial Evidence

The Testimonial Evidence screen allows users to capture information for testimonies identified as evidence for the investigation.

 

Note: Mandatory fields are marked with an asterisk. In addition, some fields are conditionally mandatory, or in other words, become mandatory based on the response to a previous field. All mandatory fields (including conditionally mandatory fields) must be completed on a screen.

Field Name Field Description
Were witness interviews attempted and/or completed? Select from the drop-down whether witness interviews were attempted and/or completed.
Relationship to the Individual Select the relationship of the witness to the individual.
Witness First Name

Enter the first name of the witness.

 

Note: If the relationship of the witness to the individual is an other individual/consumer, do not enter the first name.
Witness Last Name

Enter the last name of the witness.

 

Note: If the relationship of the witness to the individual is an other individual/consumer, do not enter the last name.
Witness Identifier (in an effort to protect identity)

Enter the identifier for the witness.

 

Note: If the relationship of the witness to the individual is an other individual/consumer, a witness identifier is required.
Agency Title (or Family Role) Enter the agency title (or family role if the witness is a family member).
Was this person identified as an alleged target for this incident? Select whether this person was identified as an alleged target for this incident.
Type of interview Select the type of interview.
Interview Date and Time Enter the date and time the interview occurred.
Interview completed in person Select whether the interview was completed in person.
Summary of relevant information from witness statement and attempts at interview Enter a summary of relevant information from the witness statement and attempts at interview.

[ADD] Click [ADD] to enter details for a new piece of testimonial evidence. When saved, the details will be added to the grid.
[EDIT] Select the testimonial evidence in the table and then click [EDIT] to modify the entry for the evidence.
[DELETE] Select the testimonial evidence and click [DELETE] to remove the evidence from the table.
