Investigation Information

The Investigation Information screen allows users to document information about an investigation.


Field Name Field Description
Investigation Start Date Select the date the investigation started.
Investigation End Date Select the date the investigation ended.
Incident Allegedly Occurred Date Select the date the incident allegedly occurred.
Incident Reported/Discovered Date and Time Select the date and time the incident was reported/discovered.

Investigator Assigned Date and Time

Select the date and time the investigator assignment was made.
If the investigator was assigned more than 24 hours after the discovery date, then please explain If the investigator was assigned more than 24 hours after the discovery date, provide an explanation.
Information provided to the investigator at the time of the assignment Enter information provided to the investigator at the time of the assignment.
Investigatory question(s) that must be answered by the investigation Enter the investigatory question(s) that must be answered by the investigation.
Did the investigator visit the scene of the incident? Select whether the investigator visited the scene of the incident.
Date and Time of the visit Select the date and time of the visit.
