Final Mortality Review Information
The Final Mortality Review Information screen allows users to document Final Mortality Review information and indicate the approval status of the Initial Mortality Review. The Final Mortality Review is only available after the Initial Mortality Review has been submitted.
Note: Mandatory fields are marked with an asterisk. In addition, some fields are conditionally mandatory, or in other words, become mandatory based on the response to a previous field. All mandatory fields (including conditionally mandatory fields) must be completed on a screen.

| Initial Mortality Review Approval Status: | Select the approval status of the Initial Mortality Review from the drop-down list. |
| Reason not Approved: | If ‘Not Approved’ is selected in the previous question, explain why the Initial Mortality Review has not been approved. |
| Was this an unexplained death? | Select from the drop-down list whether the Death was an unexplained death. |
| Cause of Death: | Select the primary cause of death from the drop-down list. |
| If other, please specify: | If ‘Other’ is selected, identify the Cause of Death. |
| Cause of Death Comments: | Enter comments related to the cause of death. |
| Body System: | Select the body system involved in the cause of death from the drop-down list. |
| Are there follow-up actions needed as a result of this Mortality Review? | Select a new value from the drop-down list if necessary to indicate whether follow-up actions need to take place as a result of the Mortality Review. Note: The value in the field is pre-populated from the Initial Mortality Review. |
| Please indicate the ODP follow-up actions: | Select new values to indicate the follow-up action or actions needed. Note: The value or values pre-populated for this question are from the Initial Mortality Review. Additionally, if the response to the previous question is ‘Yes’, a response to this question is mandatory. |
