Medication Error Dashboard
The Medication Error Dashboard provides users with the ability to analyze metrics related to finalized medication error incidents.
The purpose of the Medication Error Dashboard is to identify providers who have a high number of medication errors, and service locations that are struggling the most with medication administration. Users can use this dashboard to provide targeted training assistance to those providers, in an effort to maintain compliance with ODP’s medication administration policies for individuals who are served under ODP waivers and programs.
The Medication Error Dashboard consists of multiple views of information contained within medication errors incidents, with each view having a specific purpose. These views are divided into Medication Error Details, Service Location Details, and Response and Prevention. Users can click any of the tabs on the top right of the dashboard to see the specific view.
The Medication Error Dashboard also provides a number of filters that users can use to filter the data that is available through the dashboard. The same filters appear on all three dashboard views, and using the filters will filter data across all of the views.
| Field Name | Field Description |
| Medication Error Details | Select the Medication Error Details view to display information specific to the medication error, including the total number of medication errors, whether the error included a controlled substance, and the frequency the involved medication was given. |
| Service Location Details | Select the Service Location Details view to display information about the Service Location and activities that occurred at the Service Location leading up to the medication error. |
| Response and Prevention | Select the Response and Prevention view to display the response to the medication error and the actions being taken to prevent future occurrences of the error. |
| Date Parameter | Select whether to display the medication error information based on the date the incident occurred or was discovered. This filter is used in combination with the Start Date, End Date, and Quarter filters. |
| Start Date | Enter the Start Date of the date range to show data for incidents that occurred or were discovered within the date range. This filter is used in combination with the Date Parameter filter. |
| End Date | Enter the End Date of the date range to show data for incidents that occurred or were discovered within the date range. This filter is used in combination with the Date Parameter filter. |
| Quarter | Select a quarter or multiple quarters to show data for the incidents that occurred or were discovered within the selected quarter(s). This filter is used in combination with the Date Parameter filter. |
| Secondary Category | Select a Secondary Category or multiple Secondary Categories to show data for incidents classified as the selected categories. |
| Waiver/Program | Select a Waiver/Program or multiple Waivers/Programs to show data for incidents where individuals were enrolled in the selected Waiver/Program at the time the incident was created. |
| Region | Select a region or multiple regions to show data for incidents based on the region of the individual’s Residential County at the time the incident was created. |
| Residential County | Select a county or multiple counties to show data for incidents based on the individual’s Residential County at the time the incident was created. |
| Provider | Select a provider or multiple providers to show data for incidents filed for the selected provider(s). |
| Provider Type | Select a Provider Type or multiple Provider Types to show data for incidents filed for the selected Provider Type(s). |
| Service Location | Select a Service Location or multiple Service Locations to show data for incidents filed for the selected Service Location(s). |
The Medication Error Details view displays information specific to the medication error, including the total number of medication errors, whether the error included a controlled substance, and the frequency the involved medication was given.
Note: The Medication Error Details view contains metrics for medication errors that occurred statewide. These metrics are not affected by the Provider filter, and allows users to understand how an organization compares to all organizations across the state.

| Medication Errors | This metric displays the total number of medication errors that have been filed and finalized. Note: This metric is displayed at the statewide level and for the selected provider(s). |
| Involved Controlled Substance | This metric displays the percentage of medication errors that involved a controlled substance. Note: This metric is displayed at the statewide level and for the selected provider(s). |
| Involved Medication Frequency | This metric displays the percentage of medication errors based on the frequency that the involved medications are given to the individual, whether given routinely, as needed, or both routinely and as needed. Note: This metric is displayed at the statewide level and for the selected provider(s). |
| Incident Count by Secondary Category | This metric displays the count of medication errors by Secondary Category. Note: This metric is displayed at the statewide level and for the selected provider(s). |
| Number of Medication Errors by Number of Individuals | This metric displays counts for the number of times a medication error occurred for a unique individual and groups the counts into predetermined groupings. |
| Linked Incidents | This table displays information about incidents that are linked to medication error incidents. Note: Linked incident information is taken from the most recently submitted Final Section document of the linked incident. |
The Service Location Details view displays information about Service Locations and activities that occurred at the Service Locations leading up to the medication error.

| Incident Count by Day of Week | This metric displays the count of medication errors by the day of the week when the medication error occurred. |
| Incident Count by Time of Day | This metrics displays the count of medications by the time of day when the medication error occurred. |
| Percentage of Incidents Involving Staff Working Longer Hours | This metric displays the percentage of medication errors that occurred when the involved staff were working longer than their regular work hours. |
| Percentage of Incidents Occurring Over Multiple Consecutive Administrations | This metric displays the percentage of medication errors that occurred over multiple consecutive administrations. |
| Incident Count by Length of Time Giving Medication (Years) | This metric displays the count of medication errors by the length of time in years that the involved staff has been giving medication to individuals. |
| Incident Count by Number of Individuals Receiving Medication at Time of Error | This metric displays the count of medication errors by the number of individuals who were receiving medication from the involved staff around the same time that the medication error occurred. |
| Incident Count by Staff Position Giving Medication | This metric displays the count of medication errors by the position of the involved staff member. |
| Incident Count by Number of Medications Involved | This metric displays the count of medications errors by the number of medications that were involved in the medication error. |
| Incident Count by Number of Medications to be Given at Time of Error | This metric displays the count of medication errors by the number of medications that the involved staff was supposed to give to the individual at the time when the medication error occurred. |
| Incident Count by Number of Medications to be Given to Individual Daily | This metric displays the count of medications errors by the number of medications that are to be given to the individual daily. |
The Response and Prevention view displays information about the response to the medication error and the actions being taken to prevent future occurrences of the error.
