Document Upload
The Document Upload screen allows users to upload relevant documentation to an incident and classify the document prior to the upload.
Users are allowed to upload the following file types:
• .doc
• .docx
• .xls
• .xlsx

Field Description |
Document Type | Select the document type from the drop-down list. |
File Name | Click [SELECT FILE] to select the file to be uploaded.
Notes: The name of the selected file will appear in the table as a hyperlink after the file is uploaded. Click the File Name hyperlink to view the uploaded document. |
[UPLOAD] | Click [UPLOAD] to upload the file to the incident.
Notes: For ODP-BSASP, ODP-ID/A, and OMHSAS incidents, documents can be uploaded to an incident until the latest Final Section is submitted. Only users from the filing organization are able to upload documentation. For OLTL incidents, all users can upload documents throughout the incident lifecycle, including after incident closure. |
[DELETE] | Click [DELETE] to remove the uploaded document from the incident.
Notes: Uploaded documents can be deleted after Final Section submission. Uploaded documents can be deleted only by the user who uploaded the document and by Incident Administrators. |
