Investigation Assignment

The Investigation Assignment screen is the first screen in the management review document. If the management reviewer determines that an investigation is required and assigns an investigator, EIM will automatically generate an investigation document when this screen is saved. Once the investigation has been completed, the reviewer will be able to mark the investigation document as approved or not approved.  If an investigation is not approved, a new investigation document will be generated when the management review is submitted.

Note: Mandatory fields are marked with an asterisk. You must complete all the mandatory fields on a screen. In addition, some fields become mandatory as a result of your answer to previous fields.

 

Field Name

Field Description

Does the incident require additional investigation?

 Indicates if additional investigation is required.

Proceed with Investigation?

 Select Yes or No from the drop-down list to indicate if the incident requires additional investigation actions.

Assigned Investigator

 If required, type the name of the individual assigned to investigate the incident and select the name from the drop-down list.
 
Note: If an assigned investigator no longer has the appropriate role to perform investigations, their name will appear as a label next to the textbox. A new assignment can then be made if necessary.

Investigation Approval Status

 Select the appropriate approval status of the investigation from the drop-down list.

If the investigation was not approved, please provide comments

 Enter information about the investigation if it wasn't approved.

Indicate Investigation Determination

 Select the investigator's determination of the incident from the drop-down list.

Concur with provider investigation?

 Select Yes or No to indicate if the investigator's determination is comparable to the provider's determination.

As a result of the investigation, is this case a founded abuse or neglect?

 Select whether the incident was as a result of abuse, neglect, both or neither from the drop-down list.
